Part of what makes Premier Integrative Functional Medicine different is the fact that it is truly all-encompassing. We will leave no stone unturned to get to the bottom of your problem. As such, the forms and history are fairly extensive. Please take the time to fill out the forms as completely as possible before your first appointment in order to minimize time in the office and have a more complete picture.
